Description of Services
Through its network of web properties, RiverLink Health provides you with access to a variety of resources, collectively ("Services"). The Services, including any updates, enhancements, new features, and/or the addition of any new web properties, are subject to the TOU. The details can be found in the downloadable document below.
RiverLink Health is an HMO plan with a Medicare contract. Enrollment in RiverLink Health depends on contract renewal. This information is not a complete description of benefits. Contact the plan for more information. Limitations, copayments, and restrictions may apply. Benefits, premiums and/or copayments/coinsurance may change on January 1 of each year. The formulary, pharmacy network, and/or provider network may change at any time. You will receive notice when necessary. You must continue to pay your Medicare Part B premium. Medicare beneficiaries may also enroll in RiverLink Health through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov. Medicare evaluates plans based on a 5-Star rating system. Star Ratings are calculated each year and may change from one year to the next. Call Customer Service at 1-866-329-3970 (TTY 711) Monday – Friday 8 am to 8 pm, and Monday – Sunday 8 am to 8 pm from October 1 to February 14. You may reach a voicemail on weekends and holidays; please leave a message and your call will be returned the next business day. This information can be made available in other formats or languages. Please call Customer Service for assistance. A sales person will be present with information and applications. For accommodation of persons with special needs at sales meetings call 1-866-329-3970 (TTY users call 711). Calls will be answered by a licensed insurance agent/broker.
